Designing Cost-Effective Telemedicine Camps for Underprivileged Individuals in Less Developed Countries: A Decomposed Affordance-Effectivity Framework

Abstract: Free telemedicine camps (telecamps) are emergent joint initiatives of healthcare organizations, national and local governments, and not-for-profit nongovernmental organizations (NGOs) with the goal of alleviating the health divide for underprivileged individuals in rural areas of less developed countries. Our study seeks to understand the effectiveness of physician-patient communication at telecamps with several salient characteristics: rural underprivileged patients, physicians in remote cities, and frugal telemedicine technology—specifically, videoconferencing—deployed in Hospitals on Wheels and appropriated by operators. We adopt a multiple-actor perspective, propose a decomposed affordance-effectivity framework, and combine variance and process perspectives to examine the phenomenon of interest. We collaborated with Apollo Hospitals, a leading hospital system in India, and collected multisource data from two major telecamps in rural South India. Based on an analysis of survey data from 216 telecamp participants through a variance perspective, we found support for the fit of patient-perceived media richness with two contingency factors—(1) disease diagnostic complexity and (2) patient healthcare needs fulfillment—in influencing patient satisfaction with teleconsultation. Based on an analysis of 46 sessions of teleconsultation video archives through a process perspective, we found that technology appropriation is realized through verbal and nonverbal communication events between patients and physicians, with on-site operators playing multiple roles that serve as “compensatory user effectivity.” Our findings yield theoretical and practical implications for how effective telemedicine encounters using frugal technologies can be designed in combination with other cost-effective support personnel resources to broaden healthcare access for underprivileged individuals in less developed countries and, more broadly, to actualize technology affordances in use situations involving multiple actors. Abstract: Information technology (IT) is an important enabler of innovative learning scenarios. Today, numerous IT-enabled learning scenarios – often referred to as technology-mediated learning (TML) – such as massive open online courses, are applied worldwide across different cultures. However, little insights are available regarding the appropriation of TML and how culture influences the appropriation process. Based on adaptive structuration theory and espoused cultural values, we develop and evaluate a theoretical model capturing the cultural effects in the context of TML appropriation. By means of structural equation modeling and a partial least squares approach, we research the moderating role of espoused cultural values and its influence on TML appropriation. The results show that faithfulness, comfort with technology, and collaborative learning appropriation significantly affect learning success as appropriation outcome. Moreover, the results indicate that espoused cultural values influence the appropriation of TML. The present paper thus theoretically contributes to the scientific discussion concerning TML appropriation and the impact of culture on the deployment of IT while also making a practical contribution by deriving implications for TML application across different cultures. Abstract: Research on IS success and IT adoption has shown that object-based beliefs about IT systems have a profound impact on subsequent IT usage. However, we still lack knowledge on and need to identify antecedents and determinants of object-based beliefs in order to understand how the belief formation process works and how it can be influenced. Our research builds on and extends Markus and Silver's (2008) concepts of functional affordance and symbolic expression to examine how IT-related factors influence the formation of beliefs. To test our research model, we surveyed 183 users of a student information system. The proposed model was supported, offering evidence that values, meaning, and functional affordances provided by an IT system positively affect information quality and system quality. Collapse

Abstract: Based on the theory of effective use and adaptive structuration theory, we propose that employees’ system exploration behavior can be affected by factors related to three major components: task, system, and organizational environment. Specifically, we examine how task characteristics (job autonomy and task variety), system complexity, and innovation climate jointly affect employees’ exploration, which, in turn, leads to extended use of enterprise systems. A field survey of enterprise resource planning (ERP) users yields several interesting findings. First, job autonomy and task variety directly enhance system exploration. Second, system complexity plays a moderating role by strengthening the relationship between job autonomy and exploration and weakening the relationship between task variety and exploration. Third, innovation climate, also acting as a moderator, strengthens both the impact of job autonomy on exploration and the impact of system exploration on extended use. This research contributes to information systems (IS) research by theoretically articulating that system exploration is subject to the simultaneous influences of task, system, and organizational environment factors and empirically testing these factors’ main effects and interactions to shed new light on system exploration research. It also contributes to IS practice by suggesting that organizations could enhance employees’ system exploration and facilitate the transition from exploration to extended use by increasing job autonomy and task variety, designing personalized training programs to reduce system complexity, and developing organizational climates that foster innovations. Abstract: Instant messaging (IM) technologies are being rapidly deployed in the workplace. Current studies largely focus on the adoption of IM and how IM is used. Little research has been conducted to understand the potential impact of using IM in the workplace. This paper theorizes and empirically tests how the frequency of IM interruptions and the position power of message sender could interact with an individual’s polychronic orientation, that is, multitasking preference, and jointly influence employee satisfaction and subjective task complexity. The present study illustrates that polychronic knowledge workers are more satisfied with the multitasking work process deploying IM technology than monochronic ones. In addition, the effect of interruptions is dependent upon an individual’s polychronic orientation. The increase in interruption frequency only reduces the process satisfaction of monochronic individuals but not polychronic individuals. Further, the polychronic orientation of message receivers also influences how they process information. When IM messages are sent from their supervisors, monochronic individuals tend to prioritize tasks and perceive a lower level of overall task complexity. The information processing of polychronic individuals seem to be less influenced by the position power of message sender. Collapse

Abstract: Prior research on technology and team performance concludes that the fit of the technology to tasks influences team performance. It also suggests that the way teams appropriate technology influences performance. This research examines how fit and appropriation (from the Fit Appropriation Model) influence performance over time. Initially, the results show that fit better predicted performance; teams using poor-fitting technology performed worse than teams with better fitting technology. However, over a short time period (two days in this study), this initial fit no longer predicted performance; performance of teams using better fitting technology remained constant while teams using poor-fitting technology innovated and adapted, improving performance. There are two key findings from this study. First, fit can predict team performance soon after technology adoption, but initial assessments of fit are temporary as teams innovate and adapt; thus, our current theoretical models of fitting technology to a task likely will not be useful beyond the first use. Second, teams should understand how to better adapt existing technology and work structures. Because our current theories of task-technology fit failed to predict performance beyond the first use of technology, we believe that this calls for a reconsideration of what fit means for teams using technology. Abstract: A number of theoretical models have been presented in group support systems (GSS) literature, which suggest that various GSS structures such as anonymity and simultaneity, influence group interaction, which in turn influences group productivity and meeting outcomes. Examples of such theories include the adaptive structuration theory and the balance of forces model and they could generally be described as dynamic or procedural in nature. Much of the empirical research that tests such theories, however, is deterministic in that it often compares final outcomes between various levels of technological support without measuring and testing (1) the influence that the technological structures have on group interaction and group dynamics, and (2) the corresponding influence that group interaction has on meeting outcomes. This paper reports a study that examines the validity of such dynamic theories by examining the relationships between GSS structures, group dynamics, and meeting outcomes over time. Four process constructs (production blocking, free riding, sucker effect, and evaluation apprehension) and three meeting outcome constructs (group cohesion, affective reward, and self-reported learning) were initially selected for the study. Structural equation modeling was used to analyze longitudinal survey data gathered from an experiment conducted with naturally occurring groups. The model tested was found to be valid and GSS was found to be effective in reducing process losses. However, the findings also revealed that process losses vary in the degree to which they influence meeting outcomes and certain meeting outcomes, such as affective reward, were found to be heavily influenced by other meeting outcomes, such as group cohesion and self-reported learning.
