Abstract: Opioid Use Disorder (OUD) is a problematic pattern of opioid use leading to problems or distress. It can lead to higher healthcare costs and serious harm to patients. Patients have vulnerability to OUD based on their history, genetic makeup, current environment and stressors, medical condition and type of opioid prescribed. The number of people in the US with OUD exceeds 2 million and the total cost is approximately $100B/year. In this study, we present a theory-based design approach and three interventions for OUD. More specifically, we present the following techno-behavioural interventions: (a) decision support for healthcare professionals, (b) reminders, monitoring, and interventions for patients and (c) group support for patients. We have developed an analytical model which can be used to study cost of healthcare and interventions, and preferences of patients using utilities. We also explore the use of financial incentives for increasing the adoption of interventions. We found that interventions can lead to considerable cost savings even for mild to moderate OUD and these savings can translate into significant incentives for patients to continue the interventions. These results will help healthcare professionals, decision-makers, and researchers to explore various interventions for different patients with OUD. Collapse

Abstract: Opioid Use Disorder (OUD) has become a major public health challenge. There have been several interventions, including those based on health-IT, proposed recently. There is a major need to study these interventions. We are interested in exploring how different IT-based interventions impact opioid related Quality of Life. We developed a model using Markov chain for three different states in OUD. The model and results can lead to better decision making by healthcare professionals, patients and insurance companies. More specifically, the proposed model and results can help in (a) whether to prescribe opioids to different types of patients, (b) what IT-based interventions are suitable with an opioid prescription, and (c) how patients and healthcare professionals can select an intervention out of multiple available interventions.Keywords IT-based interventions, opioid use disorder, Markov model, evaluation. Abstract: The globally increasing prevalence of childhood obesity is one of the most serious public health challenges of the twenty-first century. Due to the need for multi-professional therapies that require a high amount of personnel and financial resources, IT-supported interventions promise help. So far, meta-studies show their limited impact on health outcomes. This work presents therefore a design theory that helps constructing health information systems (HIS) that positively impact the performance of obesity expert and children teams. Team performance is measured through self-reports, patients’ adherence to therapy and positive health outcomes. In order to assess the utility of the proposed design theory, its underlying design process was adopted by an interdisciplinary team of therapists, patients, their parents, IS researcher and computer scientists. This team developed and evaluated several HIS services collaboratively over the course of two years. Results of this design process show first evidence of the utility of the HIS design theory. However, challenges with regard to the design process still exist and are discussed. Collapse

Abstract: To keep healthcare costs under control, a high-level of medication adherence, or compliance with medication regimen, must be achieved. The multifaceted nature of medication adherence, due to a large number of underlying factors, presents several critical challenges including how to evaluate the current level of adherence, how to improve and how to maintain the required level of medication adherence, especially for long-term chronic conditions. Several interventions to improve adherence have been proposed in the healthcare literature, however these are complex, costly and difficult to implement. It is also not clear which ones would be effective at what levels of adherence in what conditions and types of patients. Therefore, there is a need to model, evaluate and compare the interventions individually as well as in combinations for their impact on medication adherence. To address this, the design and evaluation of smart medication management system (SMMS) for improving medication adherence are presented in this paper. We also present an analytical model for evaluating the medication adherence using multiple interventions that are supported from SMMS, namely context-aware reminders, improved scheduling of medications, and support from healthcare professionals. The performance results show that very high medication adherence is achievable by SMMS for single and multiple medications even for patients with mild cognitive deficiency. Several powerful “composite” interventions are also proposed and evaluated for medication adherence. It is also shown that the total healthcare savings are significant even for slightly improved medication adherence. With higher hospitalization cost, the savings due to improved medication adherence become even more significant. The proposed work forms the basis to design personalized interventions to patients for improving medication adherence in different surroundings. Collapse

Abstract: A significant drawback of communications between patients and health professionals is their restriction to face-to-face encounters within healthcare institutions. This limits the support health professionals can provide to ensure patient adherence, which is a significant contributor to therapeutic outcome and overall healthcare expenses. Pharmacist-patient health information systems (PPHIS) have the potential to address existing non-adherence behaviors by enabling pharmacist-patient communication over the time of therapy. Due to the lack of prior research, design principles for PPHIS are derived from the information-, motivation-, and strategy model [4] and feedback from pharmacists in 21 Swiss pharmacies. To demonstrate the feasibility of the design principles, we implement and preliminarily evaluate a PPHIS. Collapse

Abstract: Designing Ubiquitous Information Systems (UIS) for supporting complex everyday situations requires extended design methods and models. To address this, we introduce design models as a special class of conceptual models that support design processes for IS in general and UIS in particular. Design models for UIS are concise abstract conceptual models of key situations that conceive social, informational, physical, and service properties as requirements for targeted Information Systems. In this paper, we show how design models can be derived from narratives of key situations, guided by information systems ontologies, and domain specific input theories. This conceptual level offers an innovative approach for designing Ubiquitous Information Systems (UIS) in general and Healthcare Ubiquitous Information Systems (H-UIS) in particular. The proposed design approach is applied for developing H-UIS for reducing Adverse Drug Events (ADEs) by improving the communications between patients and healthcare professionals. Using the domain specific input theory of Health Promotion Model (HPM), the modeling guidelines for design models are generated and utilized in the development of a H-UIS to avoid ADEs. The H-UIS improves several processes involving information technologies for healthcare professionals. The system is empirically evaluated and results show high level of interest among users and initial positive adoption effects. Abstract: The development, adoption, and acceptance of eHealth systems that change and improve patient self-care have been promising, but the results have been mixed and the work mostly atheoretical. In this paper, we respond to this opportunity by developing and assessing an eHealth system for newly diagnosed type 2 diabetes patients. Study participants used the eHealth system for a 12-month period after diagnosis in an attempt to acquire an understanding about their diabetes, develop self-care activities (e.g., blood glucose testing), and improve their biomedical outcomes. Drawing upon theories and methods from information systems and upon the Precede-Proceed model of health promotion planning, we explored the double adoption of eHealth technology and its antecedents, self-care practices and their antecedents, and improvements in biomedical outcomes important to long-term diabetes health. Path model results indicate important implications for information systems, eHealth, and health promotion practice and research, which are discussed. Collapse
