Abstract: Multisourcing—the delegation of interdependent tasks to multiple vendors—is receiving increasing attention in practice and in research. Yet, we know little about the circumstances under which organizations choose multisourcing. In this paper, we draw on incomplete contracting theory and the knowledge-based view to explain multisourcing decisions in application projects. We test our model using a comprehensive dataset of 1093 sourcing decisions made by Swiss public organizations. The results provide strong support for the model. We find that clients choose multisourcing more frequently when (1) the project is large, (2) the software is client-specific and the project is large enough, (3) client and vendor lack joint experience, (4) the client seeks knowledge, (5) the technology is not proprietary, and (6) the client is experienced in outsourcing. While these findings support common views that clients choose multisourcing in response to opportunistic threats and to knowledge needs, the findings also shed light on prerequisites for multisourcing. Abstract: Recent healthcare reform has focused on reducing excessive waste in the U.S. healthcare system, with duplicate testing being one of the main culprits. We explore the factors associated with duplicate tests when patients utilize healthcare services from multiple providers, and yet information sharing across these providers is fragmented. We hypothesize that implementation of health information sharing technologies will reduce the duplication rate more for radiology tests compared to laboratory tests, especially when health information sharing technologies are implemented across disparate provider organizations. We utilize a unique panel data set consisting of 39,600 patient visits from 2005 to 2012, across outpatient clinics of 68 hospitals, to test our hypotheses. We apply a quasi-experimental approach to investigate the impact of health information sharing technologies on the duplicate testing rate. Our results indicate that usage of information sharing technologies across health organizations is associated with lower duplication rates, and the extent of reduction in duplicate tests is more pronounced among radiology tests compared to laboratory tests. Our results support the need for implementation of health information exchanges as a potential solution to reduce the incidence of duplicate tests. Collapse

Abstract: The Health Level 7 Reference Information Model 3.0 (HL7 RIM) has been developed to describe all of the data for health care systems. We describe a process followed to integrate the HL7 RIM into a clinical Laboratory Information Management System (LIMS). The methodology included interviewing users, reviewing data from clinical studies, developing a look-andfeel prototype of user-interfaces, development of an entity-relationship (ER) diagram based solely on clinical system requirements, evaluation of the HL7 RIM, integration of HL7 RIM into a revised ER diagram, and implementation of a database and web-based LIMS prototype. Substantial difficulty was encountered due to the abstract nature of the HL7 RIM and the inconsistency between the HL7 RIM and analysts’views of requirements for the LIMS. The results indicate that a methodology for developing health related systems compliant with the HL7 RIM may benefit from beginning with the standard and working toward the needs of the system versus beginning with the requirements for the system then integrating the standard.
